Information needed to look up your reinstatement requirements online:
• Name
• Birthday
• Know your Driver’s License Number or State ID Number
• Have a cell phone OR email you can access while completing the online form.

It is advised to get an appointment.
They can also help with a payment plan.
HICKORY HOLLOW IS THE ONLY DMV LOCATION that serves in-person reinstatement in Middle TN.
